Induction of New Members set on July 30

The PICE Iloilo Chapter will be holding an induction of new members on July 30, 2022, 6:00 PM at Smallville 21 Hotel, Mandurriao, Iloilo City. Register at: https://bit.ly/ILOPICE220730

The induction of new members will be a face-to-face activity. To ensure the safety of all participants, only those who are fully vaccinated are allowed to join.

Please have the following documents ready before proceeding in this registration:

(1) PICE Membership Application Form (may be downloaded at https://drive.google.com/file/d/1aHRrCOgJrBHu3z0O7O3jpALk5iSok366/view?usp=sharing )

(2) 2×2 ID Picture

(3) Proof of Payment (Minimum of PHP3,500.00 via GCash Account No. 0919 684 3484. The said amount covers the membership fee to PICE, dinner, souvenir program, pins, hardhat, and pin. For those with additional guests to attend the event, an additional PHP750.00 will be charged per pax)

(4) PRC ID

(5) Proof of Full Vaccination (at least one dose of Jansen and two doses for other brands. The proof may be the vaccination card or the vaccination certificate generated from https://vaxcert.doh.gov.ph/ )

Also note that the following hard copies must be brought to the event upon entry:

(1) PICE Membership Application Form
(2) 2×2 ID Picture
(3) PRC ID

All attendees must be in Business Attire.
